Why schedule a consultation?

Whether you're following up on long-term care, looking for a second opinion, transferring care to our office, or just wanting to meet with one of our physicians before scheduling a procedure, come see us for a consultation. We’ll help answer your questions and determine the next best steps for your health.  

What to expect and what to bring:

Your provider will review your medical history, family history, medications and current symptoms. Please bring an up-to-date list of any medications you’re taking and the dosage for each. If you are coming to us for a second opinion, please ensure we have a copy of your medical records for review. We will work closely with you to develop a diagnostic plan and/or treatment plan to accommodate your specific needs.
