In order to make referrals and communication with our practice as easy and convenient as possible, please select one of the following options:

Telephone Referrals

Please call (816) 561-2000, select Option 2 then Press 5 to reach our scheduling department.

Fax Referrals

To download our referral form, click here. Please complete and fax to (816) 931-1758.

We will contact your patient within 24 hours of receiving your referral request. You will receive a fax notification if an appointment was not successfully scheduled.
